Proof of dental degree (Doctor of Dental Surgery - DDS - or Doctor of Dental Medicine - DMD) from a Commission on Dental Accreditation (CODA) accredited U.S. or Canadian dental school within the past 12 months of application submission. 2.

• Pass a regional clinical examination that includes competencies in endontics, fixed prosthodontics, and restorative dentristry. 5.
• Proof of current certification in Basic Life Support (BLS) Continuing Education Continuing education must be completed every two years and is due on odd years.
• During this time period dentists must complete sixteen hours of continuing education, including 3 hours related to prescribing controlled substances.
